6. Photocopy of the certificate of completion or participation in training that are relevant to the position you are applying for (1 copy); and
7. For Laboratory Technician II: Original copy of the Certificate of Employment with a list of duties and responsibilities from previous/current relevant job experience (2 copies); and
8. For promotion: certified photocopy of issuance’s, documents or any proof of your involvement in other activities and outstanding accomplishments which includes awards received from different recognized and accredited international, national, regional, and/or provincial/municipal/city organizations/institutions (1 copy).
